<p>Real beauty is timeless and has an instant connection with the deeper parts of our brain. When we think about beauty, we often assume it’s only subjective and based on the eye of the beholder. Although we all have different likes and dislikes, we also have a natural, hard-wired, objective affinity to certain appearances. When we analyze those appearances, we can see they typically have certain things in common.</p>
<p>Interestingly, these things in common have a mathematical basis and form the objective basis by which we appreciate beauty. True beauty has a certain harmony to it based on containing certain mathematical ratios that positively influence our subconscious mind.</p>
<p>The ratio I’m speaking about is one based on the research of a mathematician in the 1200’s named Leonard Fibonacci, and is the balance between the number 1 and the number 1.618. The ratio of these numbers has its basis in the Renaissance. During this time, it was realized that when things, including our faces, are in harmony with these ratios, they are perceived as more attractive and harmonious.</p>
<p>As I described in a prior blog—<a title="Beauty. What Is It and What Is it Good For?" href="http://www.adamscheinermd.com/beauty-what-is-it-and-what-is-it-good-for">Beauty: What Is It and What Is It Good For?</a>—Leonard discovered a sequence of numbers where when one number was divided by the preceding it came up with the ratio of 1:1.618. This ratio was noted in beautiful things in nature such as the flower.</p>

<p>And a curve described by these numbers was seen in nature in the beautiful shape of a nautilus.</p>

<p>If you look closely, you can see that each sequence of blocks further divide themselves according to the ratio of 1:1.618.</p>

<p>This ratio has applications in facial beauty as well. Our subconscious appreciates it when a face has these ratios and balance to it and tends to view those faces as more attractive and approachable.</p>
<p>Take a look at this patient pictured below, and consider what your mind thinks of her before photo on the left.</p>

<p>Let’s apply the ratios to this example to understand why we appreciate the right photo as compared to the left photo.</p>

<p>I suggest that we intrinsically like the photo on the right because her brow is higher and shows a curve that is congruent and in harmony with the ratios I wrote about earlier.</p>

			<content:encoded><![CDATA[<p>As we age, we discover that one of the most noticeable changes to our appearance involves the eye area. Perhaps the most common complaint I receive as a specialist in cosmetic eyelid rejuvenation is that my patients have heard from others that they look sick or tired, even when they are feeling well and rested.</p>
<p>The origin of such unwanted comments is typically the nonverbal messages the lower eyelid region sends as we age.  This area can be affected by many different factors which each contribute to the appearance of dark circles under the eyes—the first is the convexity or concavity of the lower eyelid fat. If the lower eyelid fat protrudes into the lower lid region, it often catches overhead shadows.</p>
<p>Take a look at the shadows demonstrated by the blue arrows in the photo of this patient whom I helped with my lower eyelid rejuvenation surgery.</p>

<p>Because most light is above us in our world (consider sunlight outside and room lights inside), this light is likely to cast a shadow over the protruding fat that becomes visible on the skin under the lower eyelid fat. The result of the shadowing is unattractive, dark circles, sending a message of fatigue or illness.</p>
<p>A converse problem relating to lower eyelid fat is rare but can occur occasionally and involves the lack of fat in the lower eyelid region, causing a sunkenin appearance that similarly catches and displays shadows in the lower eyelid skin. This is the second cause of dark circles under the eyes and can be seen in the case of this patient who had sunkenin circles, which caught shadows as shown by the blue arrows, and made the eyes appear to have dark circles.</p>
<p>I helped him by injecting Restylane® along the bone in this region to raise the skin and decrease the shadowing.</p>

<p>The third factor contributing to the appearance of dark circles under the eyes is dark pigment, which can deposit in the skin in the lower eyelid region. This is usually the result of sun exposure over a lifetime.</p>
<p>Notice the before surgery photo of this patient who had multiple dark age spots and pigment in the skin of the lower eyelid, demonstrated by the blue arrows. These spots, along with the upper and lower eyelid appearance, were helped with my upper and lower eyelid rejuvenation techniques.</p>

<p>A forth factor patients experience leading to dark circles under the eyes is the underlying vessels transmitting through the thin skin in the lower eyelid region. This can be seen in the photo of this patient who had two causes of dark circles under her eyes. She had the fullness catching shadows but, as this photo demonstrates, she also had thin skin showing the pink vessels in this region as demonstrated by the blue arrows.</p>

<p>Finally, the fifth factor, which can play a role in the appearance of dark circles under the eyes, is the presence of lower eyelid and cheek swelling, known as lower eyelid, cheek festoons and malar mounds. This can be seen by the blue arrows in the photo of this patient pointing to the festoons before I helped her with my lower eyelid rejuvenation procedures.</p>

<p>These are the five most common causes of dark circles under the eyes:<br />
1. Fullness below the eyes catching shadows.<br />
2. Hollowing under the eyes catching shadows.<br />
3. Brown pigment or age spots in the skin below the eyes.<br />
4. Thin skin and redness showing through the skin in the region below the eyes.<br />
5. Festoons and malar mounds catching shadows in the lower eyelid and cheek regions.</p>
<p>While these five common causes of dark eye circles can result in less than optimal nonverbal messages, there are options available to treat them, rejuvenating the appearance and cleaningup facial messages to restore effective non verbal communication.</p>
<p>Helping my patients with these issues is a true joy. I experience great satisfaction when patients report to me after I help them that others in the world better relate to them because these various causes of dark circles have been improved.</p>

			<content:encoded><![CDATA[<h3>What Is the Difference between Lower Eyelid Bags and Lower Eyelid Festoons?</h3>
<p>Many people who undergo lower eyelid surgery to improve the appearance of their lower lids are dissatisfied afterwards because of one factor which can complicate the results from lower eyelid surgery.</p>
<p>This factor is lower eyelid/cheek festoons and malar mounds.</p>
<p>Festoons are a medical condition in which the skin of the lower eyelid becomes significantly damaged, usually by the sun. This damaged skin can result in folds in the lower eyelids and can lead to swollen mounds on the cheeks. They often occur with age and are more common in people with lighter skin types.</p>
<p>Festoons occur in many people seeking lower eyelid rejuvenation and if not corrected at the time that lower lid surgery is performed, the condition can become exacerbated and make the lower eyelids look worse after surgery.</p>
<p>Take a look at this patient before her surgery.</p>

<p>These two conditions can also be seen in the photo of another patient of mine prior to surgery. The white arrows point to the festoons, and I think you can now see the lower lid bag in the lower lid above the festoon.</p>

<p>The reason then why many people are dissatisfied after undergoing lower eyelid surgery to improve the appearance of their lower lids is the result of two main issues:</p>
<ul>
<ol>1. Swelling that occurs in the face after lower eyelid surgery often accumulates in the lower eyelid region and can take a while to resolve.</ol>
<ol>2. The subconscious brain constantly looks for balance in a face, and when lower eyelid fullness and festoons exist they are seen to belong together, as both are usually age-related phenomena. When only one is then corrected (usually the lower eyelid bags), the other is seen in even greater contrast.  The subconscious brain then focuses in on this condition and tells the conscious brain the face is out of balance.</ol>
</ul>
<p>You can see this result in the case of this patient the day after surgery, where the bags were removed first and the festoons weren’t treated until one day later. This is her picture one day after surgery, and you will notice how much worse the festoons appear in a case like this when they not corrected at the same time the lower eyelid bags are corrected. The festoons are even more noticeable now than in the photos above taken before surgery.</p>

<h3>Effective Treatment for Festoons and Malar Mounds</h3>
<p>Festoons and malar mounds have been a frustration for surgeons for many years. Thanks to the initial work of Dr. Sterling Baker (a person with whom I trained) and my own improvement and refinement on that work over the last decade, I now have a sophisticated, technically-challenging but effective treatment for festoons and malar mounds.</p>
<p>Many people, including some great surgeons, don’t fully understand or recognize the differences between these two conditions. I hope this blog article helps those reading to better understand those distinctions. The first difference is in their cause or etiology; festoons are a result of sun-damaged skin on the lower eyelid and on the cheek and are believed to be affected by underlying contrasting muscle forces over the years of a person’s life.</p>
<p>In contrast, lower eyelid bags are due to age-related fat protruding through the skin in the lower eyelid region. New research has shown that fat can grow and atrophy during our lives in asymmetric ways. The growth of fat in the orbit around the eye along with atrophy of fat in the facial regions around the eye, as well as weakness in some of the orbital containing structures, contribute the fullness we see in the lower eyelid with age.</p>
<p>For some individuals, this fat shows up at a very young age and this is believed to a result of genetic changes in the development of the eye area in those individuals.</p>
<p>The second difference is the location on the face where they form. Lower eyelid festoons can be seen primarily on the cheek with some presence in the lower eyelid region.  Lower eyelid bags are located directly below the lower eyelid lashes and are noticed to become more prominent when someone with these bags looks upwards.  Conversely, festoons are minimally affected when a person looks upwards.</p>
<p>The third difference is the feel of these conditions when they are touched. Festoons are squishy and can be made to move side to side. Lower eyelid bags are firmer, deeper in location beneath the skin and can’t be moved easily side to side when touched.</p>

<p>It’s true that typically when we first meet one another we evaluate each other’s skin tone and tend to appreciate clear, beautiful, and even skin. We then evaluate each other’s eyes and mouth regions. This happens quickly and on a subconscious basis. We aren’t being shallow; it’s just how our brains work.</p>
<p>Our subconscious then tells our conscious mind how we feel about that person based on what we see in those areas. If the skin is irregular, we may see the person as older or sick. If the eyelids are heavy or baggy, we may form the opinion that the individual is old, tired, sick, sleepy or even hung-over from too much alcohol the night before.</p>
<p>What does your mind see in the photo on the left of my patient when he first came to see me? Does your opinion change when you view the picture on the right which shows him after I performed lower eyelid rejuvenation surgery?</p>
<p>This patient was a teacher and his students often thought he was sick, fatigued or drinking too much based on the non-verbal cues from his lower eyelid fullness. These comments ceased after this area was improved with surgery. Now his students can relate to him without the eyelid fullness shaping their opinions.</p>

<p>Let’s look at another example in the picture of the woman below. What opinion does your mind form of this woman when viewing the photo on the left? Does she appear happy or stern, approachable or not?</p>
<p>What does your mind see in the photo on the right? You can clearly see she now appears more open and approachable as well as more pleasant. The only thing that changed for this patient was the heaviness around her eyes. These two photos are before (left) and after (right) my Botox Browlift procedure, which made a big impact on the way other people related to her.</p>

<p>When we look at the mouth area and see loss of volume in the lines around the nose to the corners of the mouth, and also from the corners of the mouth to the jawline and jowling in the jawline, we often assess that the person is older, sick and less vibrant in appearance.</p>
<p>Check out the additional before and after photo below. Stop and consider how your mind interprets my patient’s photo on the right and then on the left. By restoring the volume of youth to her lower face, she found that others viewed her as years younger and more vibrant.</p>

<p><strong>Examining the Impact of the Central Face</strong></p>
<p>I enjoy the fact that I can help my patients with all three of the D’s of aging with outpatient procedures that have quick recoveries. When I assist patients in these ways, the reward is twofold: the world relates to them better, and I experience great professional satisfaction and joy from helping my patients.</p>
<p>With this as a background, I’d like to share a story about a recent medical meeting I attended. At this meeting, a doctor who helped perform the first facial transplant in the United States, Dr. Daniel Alam, spoke about his experiences in this surgery.</p>
<p>He began by reinforcing what I have been saying for years about how we relate to each other as people. We evaluate each other’s key areas for nonverbal communication: the eyes and mouth region.</p>
<p>When either of these areas is made more attractive, the entire face appears more attractive. In this way, these areas hold special power in how our subconscious brains communicate with each other.</p>
<p>Dr. Alam continued to say that our personal identities revolve around these central parts of the face. He referred to research, which showed we look at each other’s eyes and mouths as we communicate with one another.</p>
<p>This research paper by Caroline Blais et al, entitled “Culture Shapes How We Look at Faces,” discussed cultural differences in how we evaluate each other, and showed that the central areas of the face absolutely dominate non-verbal communication.</p>

The doctor then discussed that in certain medical cases, the central face can become distorted. He showed photos of one of the first facial transplant patients in the United States. This patient, Connie Culp, suffered a gunshot wound to the face in a devastating case of domestic violence. Ms. Culp’s husband shot her in the face and then tried to shoot himself. Both survived but Ms. Culp was left with a massive injury to her central face. Her subsequent surgeries left her with a distorted face.</p>

<p>Dr. Alam then discussed how our identity is defined by these central facial areas, and how, in cases like Ms. Culp&#8217;s, her injury made it difficult for her to be accepted by others. In our society, people may have trouble being friends with Ms. Culp based on her appearance.</p>
<p>Although we would like to believe we relate to others based on what they say and do, the nonverbal language that influences our subconscious mind, does indeed influence the way we interpret each other.</p>
<p>We yearn as social human beings to relate and interact with one another, and Ms. Culp&#8217;s injury made it difficult for her to build new relationships and friendships. In fact, Ms. Culp shared that in the deformed state, children would often run away from her and call her a monster.</p>
<p>Children don&#8217;t have the mental sophistication to allow them to overpower their initial subconscious impressions. But what they see, we subconsciously see in the same way.</p>
<p>Fortunately, Ms. Culp lives in this age of incredible medical advances. She underwent one of the first facial transplants in the world. Now, she has a much easier time living life day-to-day, interacting with others, and she is experiencing more acceptance from people around her. These days, children don&#8217;t run away or call her names. The people she encounters relate to her in a more positive way.</p>

<p>The sophistication of these procedures will continue to advance and give hope to others with serious facial injuries like Ms. Culp.</p>
<p>What I found so intriguing about this procedure is why it was performed in the first place. The surgeon performed the procedure in order to help this person relate better to others in the world. This is the<strong> same</strong> reason I love doing what I do. Helping my patients restore, reveal and reclaim their natural beauty, helps them relate better to others in the world as well. It&#8217;s powerful for our patients, professionally fulfilling for us as doctors, and it’s all along the same spectrum.</p>
<p>When I meet new patients for the first time, I assess their situations through this lens. I consider what I can do to help them recapture their natural beauty and improve the way others relate to them. My ultimate goal is to achieve enhanced appearances for patients and, in turn, facilitate enhanced relationships to create better lives.</p>

			<content:encoded><![CDATA[<p>Sometimes it seems like the best way to illustrate the effectiveness  of my cosmetic procedures and surgeries is to share real-life success  stories from patients who are just like you. These are people who want  to make a positive change in their lives, rejuvenating their appearance  and transforming the way people view them.</p>
<p>My 5 minute nose job is  a procedure which can help many different types of problem noses with a  quick injectable office treatment. People undergoing this procedure  often experience a new lease on life.</p>
<p>I especially recall one  patient I treated with this procedure. She needed this performed on a  particularly busy day for me. I had in-office surgeries to perform, so I  scheduled this 5 minute nose job procedure early in the day. I gave her  a numbing solution to numb her nose and then let it go to work while I  went on to perform one of the in-office surgeries on my chart.</p>
<p>I  then came back and performed her 5 minute nose job. The patient was  around 60 years old and reported that her nose had always bothered her.  She wasn’t enthusiastic about surgery for her nose and thought there  were no other options. She was hopeful when she learned about my 5  minute nose job and came in for the treatment. When I finished her  treatment, I handed her a mirror to see the results for herself.</p>
<p>She held the mirror and then tears slowly started coming down her face.</p>
<p>My  staff and I were all there, and I have to say that we started feeling  teary, too. I had just helped fix a problem that had been bothering her  for over 50 years of her life.  It was a wonderful moment for me, my  staff, and for the patient. Below you will see photos of this patient:</p>

I had been thinking about her recently and  was considering a new way to use Botox to help bring out her natural  beauty. <p>In  talking with her, she shared how she recently went to a restaurant  where she would order her usual Greek salad. The person helping her at  the restaurant had known my patient for a long time, and they often  talked about their lives with one another.</p>
<p>In talking with this  person, my patient, in casual conversation, shared that she was 46 years  old. The restaurant employee stopped talking, stared at her in shock  and said, “I would have never guessed that you were in your 40’s and  kept saying to her “I had no idea, I had no idea.”</p>
<p>My patient  shared that it had been a long time since she had heard a compliment  like that, and it made her feel really good inside.</p>
<p>You see, she  was always the youngest in her grade and looked young for most of her  life, but for the last decade or so she was looking her actual age. She  was tired of people saying she looked tired.</p>
<p>After the school play  that evening, she shared with me that now when people ask how she is  doing, she might say she is tired but people tell her she may feel  tired, but she looked GREAT! .</p>
<p>This woman works in the business  world, and studies have consistently revealed more attractive people are  perceived as more confident and more articulate. This absolutely  resonated with my own research. I have to say that seeing the joy in her  face as she shared this story with me reinforced for me why I so love  helping people in this way.</p>

<p>I saw this Today Show news segment the other day:</p>

<p>I  immediately recognized this story as being a good point of discussion  for the readers of my blog. I’ve written before that the eyes are one of  the most critical areas of our face, not only for vision purposes but  also for communicating with others. Because we look at one another’s  eyes so often during conversation, when our eyes and eyelids change, it  can make a big difference in the way others perceive us.</p>
<p>The woman  from this Today Show segment stated she had a problem with congenital  eyelid drop. In these cases, the eyelid muscles don’t fully develop and  children with this condition often use their foreheads to pull their  eyelids upwards.</p>
<p>Depending on the severity of the condition, some  people with congenital eyelid drop can have their eyelid muscles  “tightened” to raise the lid. However, in more severe cases, specialists  will create a pulley system which connects the eyelids to the eyebrow  to increase the ability of the brows to control the eyelid height.</p>
<p>This  procedure is important in children because if the lids are blocking  vision, the child can develop a condition known as Amblyopia or “Lazy  Eye.” A common symptom of Lazy Eye is that the brain develops less brain  connections to an eye whose vision is blocked. If the condition is very  severe and not corrected before age 8-10, this can result in permanent  poor vision in that person’s eye for his or her lifetime.</p>
<p>In the  case of the woman from the Today Show, it doesn’t seem like the  condition was that severe, and it seemed her surgeon took tissue out of  the eyelid in an effort to raise the lid. It makes me question if she  truly had a “congenital ptosis.” In a case of true “congenital ptosis,”  this would not have been the suggested procedure.</p>
<h4>Importance of Choosing a Qualified Specialist in Eyelid Surgery</h4>
<p>As  the other doctor specialist in the news piece commented, removal of  eyelid tissue can be a “danger sign” and recipe for disaster. Although  probably well-meaning, the doctor in this case should have asked for  help from a specialist in eyelid surgery before operating on this  patient.</p>
<p>When undergoing eyelid surgery, it’s important for the  surgeon to have in-depth knowledge of the eyelid anatomy, and it is  especially helpful if they have a background in Ophthalmology itself.  Many eyelid surgeries are performed by surgeons who do not have the best  training in the eyes, which often leads to less than optimal results  for the patients.</p>
<p>Thankfully, most people do not suffer from the  problem that the woman in this Today Show segment had. However, it is  good to keep in mind when seeking eyelid surgery to look for a surgeon  who has extensive experience and knowledge regarding the area around the  eye in which he or she is operating. Choosing a qualified eyelid  surgeon is the best way to achieve the desired results in both the  appearance and function of the eye.</p>
